Summarizes How to Assign, Track, Complete and Download Forms

Forms are primarily used to assess caregiver competencies and track task completions related to training requirements. Supervisors or trainers complete these forms to evaluate employee skills or confirm task completion, ensuring proper documentation of the process.

Forms can be added, removed or downloaded as a single action from the Employee Profile or as a bulk action from the Employees PageReports can be pulled to track form status.

Managing Forms in the Employee Profile

Add a Form

1. Navigate to the Employee Profile and select the Forms tab.

2. Click Add Form.

3. Select a form from the list

Remove a Form

1. Navigate to the Employee Profile and select the Forms tab.

2. On the Forms tab, click the three-dot menu next to the form you want to remove.

3. Select Remove Form.

4. Confirm the removal by clicking Remove in the pop-up modal.

Filling out a Form

1. Navigate to the Employee Profile and select the Forms tab.

2. Click on the Form you'd like to fill out.

3. The Form will open in a new page

4. Navigate to the bottom of the page and click the Edit button to fill out the relevant details.

5. To save the changes, click the Save button at the bottom of the page.

6. Click the Back arrow to return to the Employee Profile.

Changing Form Status

Note: The purpose of the Form Status is to be used as an indicator for the admin.  It is not directly connected to the completion level of a form.  It also allows admins to be able to track, sort and filter forms by their Status. 

1. Navigate to the Forms tab on the Employee Profile.

2. Click the status dropdown next to the form you'd like to update.

3. Select Not Started, In Progress or Completed to update the status.


Downloading a Form

1. Navigate to the Forms tab on the Employee Profile.

2. Click the three-dot menu next to the form you'd like to download.

3. Select Download Form to download the PDF document.

Note: Forms can only be downloaded if they have been edited and saved at least once.


Managing Forms on the Employees Page

Filtering Employees by Assigned Forms

1. Navigate to the Employees page.

2. Open the Filters menu and navigate to the Forms filter.

3. Select the Form/s you'd like to filter by and click Apply.

4. The list will update to display only employees assigned to the selected form.


Bulk Assigning, Removing and Downloading Forms

1. Navigate to the Employees page.

2. Select relevant employees from the list and click the Actions menu.

3. Choose from:

  • Add to Form - Choose the Form and click Add to assign the selected form to all selected employees.

  • Remove from Form - Choose the Form and click Remove to remove the selected form from all the selected employees.

  • Download Forms - Choose the Form you'd like to download.  Additionally you can also filter the forms by Status if desired.  Then click Download.
